  • Abstract: This paper proposes an innovative approach to elbow relaxation, based on a dynamic brace (EDGES) employing pseudolastic alloys as actuators. These alloys can be easily deformed up to 8% and, while recovering the initial shape, they provide constant loads across a wide range of deformation. EDGES yields under involuntary jerks (thus ensuring the patient feels no pain), but, as soon as jerks fade, it starts anew exerting a constant mild force to the arm. Such a continual corrective push could be effective in improving limb posture by promoting contracted tissue relaxation without provoking spastic reflexes. Three post-stroke stabilized subjects (aged 62.3±1.5, 1 male) with an elbow flexors spasticity ranging 2-3 on the Ashworth’s scale wore EDGES for 2 weeks, at least 10h a day. No additional treatment to the examined arm was applied during this period and during the following week. Elbow posture was assessed at T0, 3h, 24h, 1 week, 2 weeks and 1 week after discontinuation of the therapy, during walking and rest, by means of both optoelectronic cinematic analysis and physiatric evaluation. Patients’ acceptability was also investigated, through a questionnaire. In all patients,a great improvement of the resting position was observed as early as 3h after the treatment (20°±5° flexion). During walking, EDGES did not constrain limb movements and it was thus very well accepted by the patients. A slight decrease in elbow flexors spasticity was also observed during the treatment in all the subjects, but this effect disappeared 1 week after discontinuation, as did posture improvements. EDGES resulted to be a good alternative to traditional static orthoses in terms of acceptability and effectiveness in improving posture and it could be very useful whenever short-term splinting is planned, i.e. during Botox therapy. Further tests will assess the long-term efficacy of EDGES. (literal)
