  • ABSTRACT: The intraaortic balloon pump (IABP) is the simplest and the most convenient form of circulatory assist, with principles that are applicable to other devices. The concept of counterpulsation was introduced in the early 1960. The fundamental basis of this concept was dependence of coronary flow on diastolic aortic blood pressure. Counterpulsation was achieved by decreasing of arterial pressure during systole and by its increasing during diastole. Thus, systolic unloading and diastolic augmentation were accomplished by using an external pumping chamber. This technique was the first mechanical form of deliberate afterload reduction. This concept brought a new era in treatment of patient with severe heart failure and cardiogenic shock, and finally led to the pioneering work by Moulopoulos (1962) who introduced counterpulsation by means of intraortic balloon.
