  • Postprandial dyslipidemia is observed in individuals with insulin resistance and could contribute to their higher cardiovascular risk. The specific role of type 2 diabetes and of adipose tissue lipoprotein lipase (LPL) have not been clarified. Ten obese diabetic, 9 obese non diabetic, and 9 normal weight control male subjects with normal fasting triglyceride levels, were administered a fat-rich standard meal (944 kcal, 57% fat, 31% CHO, 12% proteins). Fasting and postprandial blood samples were taken for lipoprotein analysis. A needle biopsy was taken to obtain abdominal subcutaneous adipose tissue 6 h after meal and, on a different day, in the fasting condition before a hyperinsulinemic euglycemic clamp was performed. A postprandial increase of large VLDL is observed in the insulin resistant conditions of obesity with and without diabetes. Compared with obesity alone, diabetes also shows an increased chylomicrons response, likely related to a posttranslational reduction of adipose tissue LPL activity. (literal)
