- AIM: The aim of the present study was to investigate whether early myocardial 99TC-tetrofosmin imaging (T1) is feasible and accurate in detecting coronary artery disease as standard delayed imaging (T2). One-hundred and twenty patients (100 male, 20 female, mean age 61+/-10 yrs, range 23 83) with anginal symptoms underwent single or double day stress-rest Tetrofosmin Gated SPECT. In all conditions T1 was obtained at 15' and T2 at 45' after injection with the same gamma camera and under the same acquisition parameters (double head gamma cameras equipped with high-resolution collimators; 64x64 matrix, 32 projections, 40 seconds projections, 8 frames/cycle protocol). Imaging quality, before and after reconstruction, was visually evaluated using a 4-point scale (0=poor; 4=optimal). Myocardial perfusion analysis was performed on a 20-segment model using both semiquantitative visual analysis (0=normal; 4=absent uptake) and QPS software (Cedar Sinai) . In both cases, reversible ischemia wa scored as summed difference socre (SDS). Coronary angiography was obtained within one month in all patients and stenosis 50% at quantitative analysis were considered significant. RESULTS Overall, image quality was scored as optimal/good in 94% of T1 and 95% of T2 (p=ns). There was a good linear relation between T1 and T2 visual semiquantitative SDS (r=0.89, p0.001) and the Bland-Altman analysis of agreement showed no significant shift from the mean of T1 and T2.
