- Abstract
- In a series of 962 pts with NHL classified according to the Working Formulation we evaluated the long-term prognostic effect of bone marrow (BM+) and peripheral blood (PB+) involvement, present at diagnosis (early) or occurring during the course of the disease (late). BM+ was detected at diagnosis in 343 patients (36%): in 213/407 (52%) low-grade (LG), in 74/299 (25%) intermediate-grade (IG), and in 56/256 (22%) high-grade (HG) NHL (LG vs. IG and HG, P<0.0001). Late BM+ was found in 39 further patients (4%): 11 LG, 11 IG, and 17 HG-NHL. PB+ was present at diagnosis in 104 pts (11%): in 16% of LG, 6% of IG (LG vs. IG P< 0l.001), 9% of HG-NHL (LG vs HG P<0.01). A late leukemic phase occurred in 69 pts (7%): 24 LG, 20 IG, snd 25 HL-NHL. The median survival was 95 mos for LG-, 52 mos for IG-, and 18 mos for HG-NHL. In LG-NHL, BM+ and PB+ BM+ at diagnosis did not affect the outcome, while the occurrence of BM+ and PB+ during the course of the disease carried a worse prognosis. In IG-NHL and HG-NHL, either initial or subsequent BM+ and PB+ were correlated with significantly shorter survival. (literal)
