Abstract

• Risk cards, an instrument for identifying the probabilities of clinical events, were developed by the National Institute of Health and have been available for some time for staging cardiovascular risk. Recently the risk of COPD and lung cancer correlated to cigarette smoking and other environmental fact was tabulated in similar risk cards subdivided according to age and sex. In Italy, 26.2% of the adult population (30.0% of mates, 22.2% of females) are smokers; 55.3% of smokers smoke more than 15 cigarettes a day. The study that provided the data on the relative and absolute risks of developing lung cancer was carried out in three geographical areas of Italy (Turin, Venice Region, Rome) on 2,912 mates (1,377 subjects and 1,535 controls) and 658 females (256 subjects and 402 controls). In mates, the increased relative risk is 23.7 times higher in smokers than in non smokers, while that for ex smokers is at an intermediate level, 11.2 times higher. The risk increases with the increase in number of cigarettes smoked: it is three times higher for individuals who smoke less than 5 cigarettes a day and rises to a level of 35.9 times higher for those who smoke more than 25 a day. In females, the increased risk is 5.1 times higher in smokers compared to non smokers, with an intermediate value for ex smokers of 2.7 times higher. The risk is four-fold for individuals who smoke 5-14 cigarettes a day and rises to 12.0 times higher for those who smoke more than 25 cigarettes a day. Regarding COPD, a male smoker aged 45 years with environmental and occupational exposure has a 20-39% probability of becoming affected by COPD within the next 10 years. At the level of the general population, this means that in the age group of population considered, in 10 years, there will probably be 20-39 cases of COPD per 100 individuals. From the risk cards presented it can clearly be seen that the greatest contribution to the incidence of lung cancer and chronic respiratory diseases over the next fews years will come from subjects who smoke and will continue to smoke. We thus hope that the present report will strengthen the efforts of the National Health Service in promoting smoking cessation programs. (literal)
