Abstract
  • The patent concerns a model to predict in a patient affected by heart failure survival free from urgent heart transplantation after 1 year, based on the assessment of cardiac conditions and comorbidities. The model was derived and validated in a large cohort (n= 2016 and n= 4258, respectively) of \"real-world\" patients enrolled at 24 european cardiology and internal medicine units. Model accuracy based on the C statistic was 0.88 (95% CI 0.86 to 0.90) in the derivation and 0.83 (95% CI 0.82 to 0.84) in the validation cohort, the highest among those previously reported. For easiness of clinical use at the bedside, the Cardiac and Comorbid Conditions Heart Failure (3C-HF) score, was built by multiplying model regression coefficients, corrected by age effect, by 10 and rounded to the nearest integer; points assigned to each dichotomous variable are summed up in a score that corresponds categories of mortality risk one year after assessment.
