- Abstract
- This report aims to investigate and map the oncology services available in the West Bank of the occupied Palestinian territory, including East Jerusalem. These services include diagnostic services, treatment services, and availability of qualified oncology staff. The diagnostic services include pathology services, immuno-histo-chemistry assays, intraoperative assays, margin evaluation, CT scan, MRI scan, digestive endoscopy, digestive endoscopy/colorectal, respiratory endoscopy (larynx), respiratory endoscopy (bronchial), ultra sound scans and nuclear medicine scans. The investigated treatment services are surgery for all types of cancer, radiation therapy with the three kinds of external, internal and metabolic-radionuclide's and systemic therapy that includes both antineoplastic therapies and biological therapies. The study included the census of all governmental and nongovernmental Palestinian hospitals in the West Bank, including East Jerusalem. The survey included specialized oncology hospitals, in addition to other hospitals whether general hospitals or maternity hospitals. The research instrument was a structured questionnaire, developed by Radiotherapy Department, the Institute of Clinical Physiology of CNR and the institute of Community and Public Health- Birzeit University, self administrated by the hospital medical director. The questionnaire included five sections: hospital general information, diagnostic services, treatment services, availability of oncology staff specialists and other information regarding seeking consultation for specific services in addition to the priority, medium and long term needs for those hospitals which want to expand their oncology services. Thirty seven hospitals out of 44 hospitals participated in the study. Most surveyed hospitals were located in Jerusalem (n=6), Ramallah (n=6) and Nablus (n=7). The second highest number was for Hebron (n=5). Bethlehem followed (n=4). Three hospitals were surveyed in Tulkarm and Qalqilia while two hospitals were surveyed in Jenin, and one hospital in Salfit. Ten hospitals were governmental while 25 hospitals were non-governmental including private and UNRWA hospitals. The results revealed the unequal distribution of oncology diagnostic and treatment services with the southern hospitals providing the least number of services. In term of diagnostic services, 6 hospitals out of 37 provided pathology services. Three of these hospitals provided immunohistochemistry assays services and intraoperative assays services. Three of the surveyed hospitals offered MRI scans services while 6 provided margin evaluation services. Only one hospital in the center of the West Bank provided a nuclear medicine scans service. The services of the CT scan (n = 15), digestive endoscopy (n = 13), colorectal (n = 13) and ultra sound scans (n = 28) were more prevalent. Most hospitals referred to other hospitals or other service providers (laboratories, or medical centers) located in the same geographical area for the unavailable services. The surgery of breast cancer, gynecological cancers, colon and rectum cancer, thyroid and gastric cancers were reported to be provided by more than 20 hospitals of the 37 surveyed hospitals, while intrathoracic cancer and brain cancer surgeries were provided by 6 and 8 hospitals respectively. The brain cancer, head and neck cancer and intrathoracic cancers surgery are mostly performed in the center of the West Bank hospitals, while the surgery of skin, breast, thyroid, gastric, colon and rectum, pancreatic, uro-genital, gynecological, soft tissues and primitive bone cancers and pediatric tumors were more prevalent in the north hospitals. Intrathoracic cancers surgery was not available in southern hospitals where brain cancer surgery was performed only in one hospital. The services for the systemic therapy (the antineoplastic and biological therapies) were prevalently provided in the north. On the other hand, there were no services for radiation therapy. Only two hospitals in the center of the West Bank had the external nuclear medicine treatment (Augusta Victoria hospital and Arab Care hospital). For the referral system of the radiation therapy, all of the referrals for radiation therapy were sent to the Augusta Victoria hospital in the center, in East Jerusalem. There was a variation between the different locations in the availability of the oncology staff and their numbers. In general, there were few oncology specialized staff in the West Bank. The number of radiologist was the same in the north and the center while a less number was reported by hospitals in the south. The number of pathologist was higher in the south with a total of 7 compared to only 3 reported in the center and 2 in the north. In conclusion, the diagnostic and treatment services in addition to the availability of oncology staff varied between the different hospitals in the West Bank and East Jerusalem. Not all hospitals were aware of the services provided by other hospitals and we hope this report will provide the needed knowledge for these hospitals. There was a need to understand this variation through in depth interviews policy makers in the PMoH and other health providers. Investment in oncology services was more in the northern governorates but not the southern ones. The quality and accessibility of these services need to be investigated. (literal)
