  • ABSTRACT: Background: asthma is a chronic inflammatory airways disease closely associated with atopic diseases like allergic rhinitis. Both diseases are increasing to epidemic proportions (allergic rhinitis: 10-50%; and asthma: 5-15%) with a reduced patients' quality of life, lower productivity and increasing medical costs. The correct management of asthmatic and rhinitic patients would be ensured by the use of ARIA (Allergic Rhinitis and its Impact on Asthma). Many studies prove that the guidelines recommendations are not or only partially applied within the clinical practice. Further, there are few studies on the control of the disease and on the treatment efficacy in the real life of the general population. Objectives: the general aim is to evaluate the applicability and the adherence/not adherence to the GINA and ARIA guidelines in different care settings and geographic areas in order to orientate AIFA regulatory activity. Methods: the project, planned to last 3 years, is framed in: a) observational studies in General Practitioners (GP), in Hospital pneumologists and allergists, in a birth cohort and in young adults; b) drug surveillance; c) educational intervention; d) prescriptions appropriateness and pharmacoeconomy. Expected results: the project will allow to provide: 1. epidemiological data on asthma and rhinitis burden (prevalence, economic costs); 2. evaluation of applicability, utility and adherence to GINA and ARIA guidelines by GPs, specialists and patients; 3. short term and long term efficacy of a Continuing Professional Development program (knowledge improvement, adherence to the guidelines, changes in disease direct costs); 4. evaluation of prescriptions appropriateness and treatment cost/efficacy ratio (beclometasone, flunisolide, budesonide, acetilcisteina, drugs associations). The following expected results will be able to contribute to AIFA regulatory activity: 1. adherence to the AIFA notes concerning anti-leukotriene and anti-histamine drugs prescriptions (notes 82 and 89); 2. quality evaluation of allergenic extracts utilized in specific immunotherapy (SIT); 3. regional reimbursement modalities for SIT; 4. use of self-inject able adrenaline in emergency department for regulation of Fast Jet Adrenaline; 5. use of unconventional therapies; 6. regional utilization of generic drugs in the place of branded drugs with expired patent.
