- Tailoring minimally invasive gynaecological surgery can be defined as the pre-emptive prophylactic operative removal of a reproductive organ or a part of it, based on molecular genetic analysis, prior to malignant transformation or when the cancer is in situ. With the advent of direct DNA analysis to detect germline mutations characteristic of a specific hereditary cancer, the use of tailoring prophylactic surgery has assumed major importance: this surgical intervention, based on molecular genetic analysis, illustrates the importance of translational research in the care of patients destined to develop a malignant disease. At present, predictive testing and prophylactic tailoring surgery are applicable for patients with specific hereditary female cancer pathologies: tailoring minimally invasive gynecological surgery may be indicated in several ovarian, mammalian, endometrial and cervical pathologies, from which the molecular pathogenesis is known and DNA-based testing is available. The role of minimally invasive surgery continues to increase, providing less invasive treatment alternatives to patients with gynecologic diseases. Intuitively, patient satisfaction has helped to drive the discipline of minimally invasive surgery: smaller incisions, less postoperative pain, and shorter hospital stays are appreciated by women suffering from gynecologic cancers. Minimally invasive gynecological surgery has been shown to be safe and feasible, while treating patients with the same efficacy as traditional open procedures. Above all, this will help to guarantee minimally invasive surgery as an acceptable approach in treating gynecologic malignancies. (literal)
