  • Background: Left bundle branch block (LBBB) is reported in a high percentage of patients (pts) with idiopathic dilated cardiomyopathy (IDC). A wide QRS and an increased adrenergic activity are associated with poorer prognosis and higher all-cause mortality rate. Relationship between LBBB and sympathetic activation is not fully understood. Aim: Of to assess whether LBBB plays any role on the adrenergic activation in IDC pts without overt heart failure. Material and Methods: 87 IDC patients (mean age: 61 years, 50 Males, LV ejection fraction - LVEF- 34±1%, LV end diastolic dimension -LVEDD- 61±1 mm (NYHA Class I-II) were enrolled into the study. 40 pts showed LBBB on the surface ECG (QRS duration 138±8 msec); 47 had normal QRS duration. All pts were under treatment with b-blockers, diuretics, spironolactone, ACE inhibitors or angiotensin-II receptor blockers. Blood samples for brain natriuretic peptide (BNP), aldosterone, noradrenaline and adrenomedullin (ADM) were taken within 1 week from a conventional echocardiographic exam. Results: The two groups of pts were comparable for age (63 vs. 60 years), male sex (22/27), type of treatment, LVEF% (35.1±1.4 vs. 36.1±1.1), LVEDD (61.1±1 vs. 60.5±0.8 mm). At 2D echocardiography, an outward motion of the interventricular septum during systole was observed by two independent observers in 38/40 LBBB pts and in none with normal QRS. Comparable BNP (124±30 vs. 118±24 pg/ml) and aldosterone (173±22 vs. 171±20 pg/ml) levels were found in the two groups; conversely, higher values of noradrenaline (599.6±65 vs 416.8±29, pg/ml, p=0.01) and ADM (23.1±1.4 vs 17.8±1.3 pmol/l, p=0.03) were found in LBBB pts. At multivariate analysis noradrenaline and ADM remained significantly associated with LBBB after correction for other clinical, therapeutic, functional and biohumoral variables. Conclusions: In IDC pts, LBBB is associated with higher noradrenaline and ADM circulating levels, independently of global LV function. These results may sugge (literal)
