  • It is known that a sizeable amount of prescribed medicines is never used, especially by subjects with respiratory diseases. Aim: to assess the temporal change of drug use in subjects with asthma/rhinitis in Italian adults. Methods: within the Po Delta and Pisa epidemiological surveys on general population samples, all the subjects who habitually used medicines (cases), and an equal number of non users (controls, randomly drawn and matched for gender and age groups of cases) were selected (baseline). Of these, 89% (N=1663, 37.2% males, mean age 63yrs) participated in a telephone interview after a mean period of 11 years (follow-up). Results: asthma and rhinitis prevalence increased, even if not significantly, between baseline ad follow-up (5.8 vs 7.1%, and 20.6 vs 23%, respectively). Habitual use of any medicine was significantly higher at follow-up than at baseline (70.1 vs 46.2%). Among the users at follow-up, the prevalence of broncho-pulmonary drugs was higher than among the users at baseline (7.5 vs 4.8%, p0.05). The same result, but non significant, was found for antiallergic medicines (2.4 vs 1.3%). New regular use of any medicines at follow-up was 60.4%. After accounting for gender, age, current/passive smoke, comorbidities, other chronic respiratory symptoms/diseases, and occupational or environmental exposure, only asthma resulted significantly related to the use of any medicines at baseline (OR 1.69, 95%CI 1.04-2.73), while the relation was significant for both asthma (2.92, 1.51-5.66) and rhinitis (1.55, 1.03-2.33), at follow-up. Specific medicines use (brocho-pulmonary/antiallergic/cardiovascular/diuretic vs other medicine/no use) was significantly associated with asthma at both baseline (3.36, 1.97-5.75) and follow-up (3.94, 2.14-7.30). Positive, but non significant, associations were also found with skin prick tests (SPT) and IgE. Positive SPT was related to higher assumption of specific drugs (1.19, 0.74-1.90 at baseline, and 1.25, 0.78- 2.00 at follow-up), and IgE concentration was positively related to higher use of both any and specific medicines at baseline (1.36, 0.94-1.96, 1.16, 0.79-1.70, respectively), and of specific drugs at follow-up (1.14, 0.78-1.66). Conclusion: in this epidemiological study, ageing, asthma and rhinitis were determinant factors of medicines consumption.
