  • Purpose. Adiponectin, a biologically active substance released by adipocytes, is considered as a new diagnostic/prognostic marker in the cardiovascular disease. This protein presents a multimeric structure; the trimeric low- (LMW), the hexameric middle- (MMW) and the high-molecular weight (HMW) isoforms have been found in human peripheral circulation. The determination of the different isoforms, specially of HMW that is an active form, could give additional information on the role of this protein. Aim. To evaluate the analytical features of a commercial system devoted to measure the adiponectin multimers in human plasma. Methods. An Elisa system (Alpco Diagnostics, US) was used to directly measure the total (T) and, after treatment with two different specific proteases, the HMW and the (MMW + HMW) forms while the concentrations of the MMW and LMW were obtained by difference. For comparison purpose, T adiponectin was also measured by a widely used Elisa (Linco Res US). EDTA plasma samples from 64 non-obese subjects were analysed and, in a subset of samples, also serum was checked. Results. HMW is the predominant isoform in plasma and accounts for 50% of T adiponectin; MMW and LMW represent each the 25% of total protein. Between-assay was 10.8 % for T and about 15% for HMW and (HMW + MMW) determination (n=10); within-assay variability was lesser than 5%. No important differences were observed between the values obtained in plasma and in serum samples (p=ns, paired t-test). As to the absolute values, T adiponectin levels measured by Alpco system were lower than those measured by Linco, (5.7±0.44 ?g/ml vs 11.6±0.92, mean±sem), although closely related (r=0.81, p0.001). This difference is probably due to the different isoform measured, dimeric and monomeric, respectively. Conclusions. These data confirm that in human plasma are present measurable amounts of adiponectin multimers and that HMW is the predominant isoform. The evaluation of the (literal)
