- Purpose. Osteopontin (OPN) is an extracellular matrix protein, with cytokine properties, apparently involved in the acute and chronic inflammatory vascular and remodeling responses to different mechanical and/or biohumoral factors. Recently, elevated OPN plasma levels were found associated with the severity and progression of atherosclerosis in coronary artery disease. No data are available on plasma levels of OPN in idiopathic dilated cardiomyopathy (DCM) and on the potential relationship with coronary microvascular function, inflammatory and biohumoral profile in these patients. The aim of this study was to measure plasma levels of OPN, IL-6, C-reactive protein, renin-angiotensin system hormones and coronary microvascular function in a population of DCM patients. Methods. OPN plasma levels of 55 no diabetic DCM subjects (38 males, 44 in NYHA class I-II, 11 in NYHA class III, LVEF% 35.2±1.3, age 51.2±1.5 years, mean±sem) and 74 healthy controls, matched for age and sex, were measured by specific ELISA. Coronary microvascular function was assessed by absolute measurement of myocardial blood flow (MBF) at rest and during pharmacologic vasodilation by Positron Emission Tomography and 13N-Ammonia as a flow tracer. Results. OPN was significantly increased in DCM patients with respect to healthy controls (508.2±36.8 ng/ml vs. 405.9±17.2, p=0.007) and significantly correlated with aldosterone (r=0.43, p=0.0001), plasma renin activity (r=0.49, p=0.0002) and with IL-6 (r=0.29, p=0.003) while no correlation was found with C-reactive protein. A significant negative correlation was found between OPN and MBF at rest (r=0.49, p=0.011), during dipyridamole stress (r=0.63, p=0.001) and with MBF reserve (r=0.49, p=0.01). Conclusions. OPN is increased in DCM despite the absence of detectable epicardial coronary artery disease. In these patients OPN is negatively related with MBF and MBF reserve suggesting a relevant role of this protein in modifying coronary microvascular properties possibly in relationship with renin-angiotensin system activation.
