  • Purpose. Adipose tissue can function as an active endocrine organ able to release many biologically active substances called adipokines. Among these, adiponectin is thought to have a protective effect on vascular atherosclerotic process by inhibiting vascular inflammatory processes and its plasma levels are reduced in obese patients and in coronary artery disease. By contrast, a high adiponectin level has been demonstrated to be a predictor of mortality in patients with heart failure and adiponectin has been found significantly increased in damaged cardiomyocytes from patients died for idiopathic dilated cardiomyopathy (DCM). These data suggest a role of adiponectin in cardiac dysfunction independently from the presence of coronary artery disease. Aim of this study was to assess the peripheral levels of adiponectin, as compared with leptin, in patients with DCM, with and without heart failure, and to evaluate its relationship with inflammatory profile and left ventricular (LV) function. Methods. The plasma levels of adiponectin and leptin were evaluated in 77 no diabetic patients with DCM (37 males, 65 in NYHA class I-II, 12 in NYHA class III, LVEF% 40.6±1.4, age 57±1 yrs, BMI 26.6±0.41 Kg/m2, mean±sem) and in 40 age- and BMI-matched healthy subjects as control, by using specific Elisa.
