  • Purpose. C-type natriuretic peptide (CNP) is widely expressed throughout the vasculature and is found in particulary high concentrations in the endothelium. Elevated CNP levels in heart failure, similarly to other natriuretic peptides, are interpreted as part of a compensatory vasodilating response counteracting depressed endothelial NO-dependent function. So far, no data are available on plasma levels of this peptide in patients with dilated cardiomyopathy (DCM) with or without overt heart failure. The aim of this study was to evaluate the relationship of CNP peripheral levels with other neurohormones, heart failure symptoms and LV function in patients with DCM. Methods. Plasma CNP levels of 51 no diabetic DCM patients (32 males, 37 in NYHA class I-II, 14 in NYHA class III, LVEF% 34.1±1.3, age 62±1 years, mean±sem) and 64 healthy controls were determined by a radioimmunoassay (RIA) after a preliminary solid-phase extraction on Sep-Pak C18. Plasma atrial and brain NP, (ANP, BNP), renin activity (PRA) and aldosterone were measured by specific immunometric assays; noradrenaline and adrenaline were measured by HPLC procedure. Results. The plasma levels of CNP in healthy subjects were 2.5±0.12 pg/ml, mean±sem, and resulted significantly increased in DCM patients with (7.0±0.9 pg/ml) or without (6.1±0.53) overt heart failure (p0.0001, patients in NYHA class I-II or in NYHA class III vs controls).
